At this time, we do not bill insurance for compounded prescriptions. Please contact your insurance company to get information on whether your plan allows you to submit paper claims for your compounded prescriptions.

If you are able to submit a paper claim, please bring or fax your claim form to the location at which your prescription was filled. We will gladly assist you in filling out these forms.
